The agency must have at least one South Carolina licensed producer, the agency can then submit the following documentation to the South Carolina Department of Insurance.
- Application for Agency License, Form 3511 [pdf 184k]
- License fee of $40
The check should be made payable to the South Carolina Department of Insurance.
- If the agency is not domiciled in South Carolina and the agency license status cannot be verified on the State Producer Licensing Database (SPLD), certification from the agency's home state must be submitted.
- Financial Interest Disclosure, Form 3600 [pdf 62k]
A financial interest disclosure form should be submitted by title agencies only.

- The requirement for an agency license does not apply to any agency owned by, and operated under, the name of an individual licensed producer, so long as no other person, firm or entity has any interest in, or affiliation with, the business of the individual producer or their agency. This type of agency is considered a sole proprietorship.
- No such agency shall be licensed under a name which is likely to lead a person of average intelligence to believe that the agency is an insurer, an agency of any government, or a club, fraternity, association, or social or military organization, or which is otherwise deceptive or misleading. Foreign corporations, limited partnerships and other entities are required by law to be registered with the South Carolina Secretary of State's Office (803-734-2158).
- Only one license is required for each agency and a separate license is not required for branch office locations unless the agency has more than one FEIN number.
Any changes made within the agency that differ from the information on the initial agency application (i.e., name change, change in ownership, licensed producer, etc.) must be submitted in writing to the South Carolina Department of Insurance.

The Department will no longer print or mail hard copies of licenses. Agency licenses must be printed via the Department's SCDOI Online Services found on our website. There is no charge for printing the licenses, and copies of duplicate or amended licenses can be obtained at anytime.

- Select "Print Agency License" under the Registered Agency Users heading.
- You must accept the License Certification Terms of Use before proceeding. If you agree with the terms, click "I Accept" at the bottom of the page.
- Key in the FEIN and Agency Code in the format as shown next the fields on the License Print Login page and then click "Login."
- Click "Print License."

- Enter the agency userid and password on the Login page and then click "Login."
- Click "Print Letter."

South Carolina law requires all licensees to notify the Department of any change in address within (30) days. Failure to notify the Department with the 30-day statutory timeframe constitutes a violation of the South Carolina insurance laws. It is the statutory responsibility of the licensees to make certain their mailing, home and/or business address records with the South Carolina Department of Insurance are up-to-date. Having inaccurate address and other contact information on file with the Department may result in licensees not receiving important notices and other communications. Licensees may be subject to administrative disciplinary action for not timely notifying the Department of an address change. In accordance with Section 38-2-10 of the South Carolina Code of Laws, individuals could be subject to administrative penalties up to $2,500 for each violation of South Carolina insurance laws.
Effective July 1, 2009, the Department will no longer accept paper requests for address changes.
