Licensing

Please complete the following documentation and submit the information as a complete package to the attention of G. Lee Hill, Jr., Chief Financial Analyst.

1. Application for Registration as a Risk Retention Group, Form 1030 
    
2. Appointment of Attorney to Accept Service, Form 1027
    
3. Biographical Affidavit, NAIC Form 11 
   
   An affidavit should be submitted on each office and director of the Risk Retention Group. 
     
4. Certification [pdf 10k]
   
   The certification must be completed by the Risk Retention Group's domiciliary insurance department.
    
5. Licensing fee of $1200.  The check should be made payable to the South Carolina Department of Insurance.
