  5. South Carolina Health Insurance Pool (SCHIP) Assessment Based Reporting Form
    Any insurer licensed to write accident and health insurance in South Carolina, even if all amounts are zero, must complete this form.  The form should be submitted to the SCHIP Administrator at BlueCross BlueShield of SC, 803-788-0500, ext. 46401 (Columbia) or 800-868-2500, ext. 46401 (Outside Columbia).
  6. Please note the "Notice of Requirements to Provide an Annual Report of Market Value of Securities as of June 30, 2012" is no longer required.

Contact an Insurance Company

  1. After reviewing the list of insurers in the Market Share Report, select the company of your choice and write down the 6-digit company code number.
  2. Click "Search for Insurance Companies."
  3. Type the 6-digit code into the South Carolina company code field and then click search. 
  4. When the result appears, click on the company's name to view demographics and license information.
  5. To view the company's complaint summaries and search by city for producers appointed by the company, click on the identified link on the right side of the page.
